  • Graduate of accredited school of nursing and licensure in California as an RN
  • Current BLS certification from the American Heart Association Or American Red Cross
  • Experience as a staff nurse for at least 2 years with one year experience in Long Term Care

Open Until Filled. This temporary position is subject to reduced hours or status in the near future. This is a exempt position.

Qualifications:

  • Skills and abilities demonstrated in the area of staff direction
  • Familiarization with LTC Comprehensive Assessment requirements

Responsibilities: 

Job Summary

The MDS Supervisor/Coordinator works under the guidance and supervision of the Director of Nurses, the MDS Coordinator will initiate and facilitate the screening, admission and comprehensive assessment process of each resident admitted. The MDS Coordinator will supervise, teach and assist the MDS Nurses. 

Duties

  • Comprehensive Assessment and Care Planning
    1. Initiates the Comprehensive Assessment Process upon admission and completes it within prescribed time frame
  • 1.Participates and coordinates the Resident Assessment Instrument (RAI) which includes the minimum Data Set (MDS) and Resident Assessment protocols (RAP’s)

    2.Participates in the Inter-disciplinary Care Plan Meeting 

    3.Facilitates care plan and initiation of short term care plan

    4.Contributes to and other I.D.T. members for care planning

    5.Maintains a MDS log with all required assessments

    6.Accurately and timely transmits assessment data complying with all state and federal regulation

    1. Communicates Relevant Resident Information

1.Reports changes in residents status to charge nurse or physician

2.Documents nursing care consistent with care planning requirements

3.Complies with hospital policy for receiving doctor’s orders, including verbal and phone

4.Keeps Unit Director appraised of pertinent information and concerns of resident

  • Exposure to blood and body fluids in Classifications I, II, & III (See Blood borne Pathogens Program, Job Classification System).
  • HBV vaccination available.

Lifting Requirements

Heavy: Frequent lifting, not more then 100 pounds (with help) and/or carrying objects weighing up to 50 pounds.
