Job Summary
The RN MDS Coordinator is responsible for directing and managing all functions of the MDS program, ensuring the accurate and timely completion of resident assessments, care planning, and reimbursement documentation. Reporting to the Executive Nurse Director, this position provides leadership to the MDS team, drives regulatory compliance, supports optimal clinical and financial performance, and collaborates with interdisciplinary departments to promote high-quality resident care and achieve organizational objectives.
Duties
- Comprehensive and Quarterly Assessment and Care Planning
- Initiates the Comprehensive Assessment Process upon admission/re-admission; Quarterly assessments for long term residents and completes them within prescribed time frame
1.Participates and completes the Resident Assessment Instrument (RAI) which includes the minimum Data Set (MDS) and Resident Assessment protocols (RAP’s)
2.Participates in the Inter-disciplinary Care Plan Meeting
3.Facilitates care plan and initiation of short-term and long-term care plans
4.Contributes to and other I.D.T. members for care planning
5.Ensure that assessments are accurate, complete and submitted within the required timelines complying with all state and federal regulation
- Initiates the Quarterly Assessment for long term residents and Care Planning
1.Participates and coordinates the completion of the Resident Assessment Instrument which includes the Minimum Data Set (MDS)
2.Participates in the Inter-disciplinary care plan meeting
3.Maintain and update long term care plans as applicable
4.Ensure that assessments are accurate, complete and submitted within the required timelines complying with all State and Federal regulations
- Maintains and updates the MDS Calendar utilizing the MDS scheduler to plan and set the Assessment Reference Dates for all required assessments and tracker forms

- Communicates Relevant Resident Information
1.Reports changes in residents’ status to charge nurse or physician
2.Documents nursing care consistent with care planning requirements
3.Complies with hospital policy for receiving doctor’s orders, including verbal and phone
4.Keeps Unit Director apprised of pertinent information and concerns of resident

Qualifications
• Graduate of accredited school of nursing and licensed in California as RN
• Experience as staff nurse for at least 2 years in long-term care and skilled nursing environment.
• Strong knowledge of MDS 3.0, RAI Manual and processes, Federal and State regulations, Medicare/Medicaid regulations, and reimbursement systems.
• Proven leadership ability to manage and oversee MDS department operations, ensuring accuracy, compliance, and timely completion of assessments.
• Experience in care planning, clinical assessments, quality measures, and regulatory compliance.
• Excellent organizational, analytical, and documentation skills.
• Strong communication and collaboration skills with interdisciplinary teams, residents, and families.
• Proficient in electronic health records (EHR) and MDS software systems.
